My target group are patient with COPD. Prevent being hospitalized by managing their COPD at home, through clinic visit and frequent assessment if their medications are being effective. <\/p>\n
I will compare my project from the previous (last quarter) hospital admission for patient with COPD.<\/p>\n
My expectation from this project is to prevent or lower the readmission rate of patient with chronic COPD in the hospital.<\/p>\n
My time frame is withing 90 days of the implementation. And see the readmission rate will go down with COPDs<\/p>\n

Projected budget is around $10,000 to buy teaching supplies such as Nebulizer machine, Aerobika, Icentive spirometer. Sample inhalers to teach how to properly use a inhalers.<\/p>\n
Why did you decide to do this project?<\/p>\n
I currently work in Pulmonary clinic in San Rafael, Ca. Most of are patient has COPD. And currently the admission rate for COPD patient in our hospital is high <\/p>\n
What do you hope to achieve by doing this project?<\/p>\n
Our patient can able to manage their COPD at home and through clinic visit. Less hospitalization and admission in the hospital. <\/p>\n
What do you need to help you complete this project? Examples may include supplies, Facility\/Administration support, etc.<\/p>\n
I need admin support, probably one nurse to teach patient how to properly use their inhalers, a respiratory therapist to help teach pulmonary hygiene and perform pulmonary function test. A nebulizer machine to show the patient how to use it, a Aerobika, and Incentive spirometer and sample inhalers for teaching.<\/p>\n
Will your project have long-term effects? If so, what are they? If none. Why not?<\/p>\n
It will have a long-term effect. By managing their symptoms at home, it will help them to be hospitalize and admission rate will go down. <\/p>\n

My project can be sustainable if patient is being cooperative about their treatment plan. <\/p>\n
My project can be evaluated by checking the hospital admission rate for patient with COPD. <\/p>\n

Yes. Patient may not be compliant with their treatment plan given by our Pulmonologist. <\/p>\n
