Although the first paragraph after the paper title is the introduction, no heading labeled \u201cIntroduction\u201d is used. Refer to your assignment guidelines for the headings to be used for the body of the paper. See the APA file in Course Resources for additional writing resources such as the APA Annotated Paper and the Chamberlain Guidelines for Writing Professional Papers. <\/p>\n<p>Background Information<\/p>\n<p>This section required paragraph format and cannot be in table format. Each section should be a paragraph:<\/p>\n<p>Demographics (gender, age, race ONLY), Past medical and surgical history, and diagnosis<\/p>\n<p>History of present illness (for example, if a client is admitted with abdominal pain, how long have they had the pain? This is the situation section (SBAR) and course of hospitalization<\/p>\n<p>Assessment (use head to toe for guidance. For example, Neurological assessment included pupils were plus two and brisk bilaterally. Patient was alert and oriented to person, place, and time.<\/p>\n<p>Labs and Diagnostic Tests<\/p>\n<p>Identify and list a summary of lab values and diagnostic tests\/procedures performed. Include the rationales for performing the tests and for results. Provide an analysis of the relationships between\/among the diagnostic tests and lab results with the critically\u2010ill individual\u2019s current condition(s). Provide reference for labs\/diagnostic values. This can be in table format, but must be properly referenced.<\/p>\n<p>Medications<\/p>\n<p>Identify and list a summary of relevant and significant medications. Indicate the trade name and generic name, the medication classification, the therapeutic use, major adverse effects, timing, dosage, route of administration, and the nursing implications. Also, include the rationale(s) as to why the medication is prescribed for treatment. For example, document the rationale for how the treatment corrects the critically\u2010ill individual\u2019s condition or a side effect of another drug that is prescribed for treatment. *This information can be presented in a table and must be cited (referenced).<\/p>\n<p>Nursing Diagnoses<\/p>\n<p>In order of priority, identify three (3) nursing diagnoses for the individualized plan of care for this patient. Two of the nursing diagnoses must address actual problems, and one of the nursing diagnoses must be a \u201cRisk for\u201d or \u201cPotential Complication\u201d (PC). For each nursing diagnosis, include three (3) nursing outcomes, three (3) nursing interventions, and three (3) collaborative interventions. *This section can be presented as a table.<\/p>\n<p>Nursing Interventions<\/p>\n<p>Describe 3-5 independent nursing care you provided in the care of the critically\u2010ill individual. Examples include, but are not limited to: activity level, position, ongoing monitoring, and nutrition (prescribed diet, tube feedings\/TPN, formula, rate, patient education, &amp; wound care etc.). Each intervention should include a rationale, evidence to support the intervention with citation, and why it is important to the client. Each intervention should be a paragraph. One way to set this up is MEAL. M=main idea (ie positioning), E=evidence (citation of scholarly article supporting this intervention. For example. According to Perry and Potter (2016), repositioning should be done every two hours to avoid impaired skin integrity.), A= analysis and L=link to patient (This client was immobile due to a fracture of the right femur and will need assistance with repositioning to avoid skin breakdown.<\/p>\n<p>Interdisciplinary Management<\/p>\n<p>Collaborative Care Management<\/p>\n<p>Identify 2-3 team members of the interdisciplinary team caring for the critically\u2010ill individual. Provide a brief description of their roles and responsibilities. Interdisciplinary team members include, but are not limited to: physician, pharmacist, lab\/diagnostic tests personnel, respiratory therapy, physical therapy, occupational therapy, speech therapy, social work\/case management, pastoral care, ethicist, etc. Each member should be their own paragraph. Nursing should NOT be in this section as that was discussed in the previous section.<\/p>\n<p>Therapeutic Modalities<\/p>\n<p>Describe the various therapeutic modalities used in the management of care for the critically\u2010ill individual. Discuss the extent of the nurse\u2019s responsibilities and skills required to manage the therapeutic modality in comparison to the responsibilities of the members of the interdisciplinary team. Therapeutic modalities include but are not limited to: oxygen therapy (mode, FiO2,), dialysis\/CRRT (settings), ventilator therapy (mode of ventilation, settings, FiO2). The rationale must be included for each modality. Each therapeutic modality should be a paragraph and have an in-text citation and reference.<\/p>\n<p>Nursing Role Reflection<\/p>\n<p>Provide a brief summary of how your role interacted with the members of the interdisciplinary team. Each section should be a paragraph and include: <\/p>\n<p>Analysis of communication style preferences among interdisciplinary team members and with the critically\u2010ill individual and family members. What is the impact of your own communication style on others? <\/p>\n<p>System barriers and facilitators. Did the organizational framework for interdisciplinary management of care facilitate or hindered the quality of care\/outcomes for the critically\u2010ill individual? What evidence-based recommendations can you make to the organizational system for enhancing interdisciplinary collaboration? Provide at evidence\u2010based literature sources to support your recommendations. Examples include SBAR, bedside rounding, etc.<\/p>\n<p>Professional Development. Based on your experience(s), write ideas for your own professional self\u2010development plan to enhance your potential for becoming an effective member in an interdisciplinary team. For example: seminars, webinars, classes, CEU\u2019s, etc.<\/p>\n<p>Conclusion<\/p>\n<p>Papers should end with a conclusion or summary. The assignment directions will specify which is required. It should be concise and contain little or no detail. No matter how much space remains on the page, the references always start on a separate page (insert a page break after the conclusion so that the references will start on a new page). <\/p>\n<p>References (centered, not bold, separate page)<\/p>\n<p>Type your references in alphabetical order here using hanging indents.
